We understand that certain students have special dietary needs, and we will do our best to accommodate those needs. A Diet Prescription Form or the provider's letterhead with special dietary needs must be signed by a licensed physician, registered dietitian, or physician assistant for students with a chronic medical condition such as diabetes, cystic fibrosis, food allergy or a disability. The Diet Prescription Form must be renewed annually.
